DENTAL BENEFITS

Full-time and regular part-time employees.

First day of the fourth month after passing probation,  provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of two consecutive calendar months during that period.  Orthodontia benefits   - Employee must be covered under dental benefits for nine consecutive months.  Benefits are effective the first of the 10th month.  Employees must complete a 90-working day probationary period.

Basic Services - 100% up to a scheduled maximum

Major Services - 100% up to a scheduled maximum

Annual Maximum per person - $1,100

Orthodontia - 70% up to $800, 5-year maximum

VISION BENEFITS

VISION SERVICE PLAN (VSP)

Full-time and regular part-time employees

(20+ Hours Per Week)

First day of the fourth month after passing probation, provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of three consecutive calendar months during that period.  Employees must successfully complete a 90-working day/480 hours probationary period.

Co-pay:  $10

Exam and lenses - every 12 months.

Frames - every 24 months.

Eye exam, Single, Biofocal, Trifocal, Lenticular, Frames:  100% of VSP allowable.

WEEKLY TIME LOSS (SHORT-TERM DISABILITY)

Full-time and regular part-time employees.

(20+ Hours Per Week)

First day of the fourth month after passing probation, provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of three consecutive calendar months during that period.  Employees must successfully complete a 90-working day/480 hours probationary period.

Weekly benefits: 2/3% of weekly earnings, $110 max. 1st 6 weeks, $160 max next 20 weeks.

LONG-TERM DISABILITY

All Full-time and Regular Part-time employees

First of month following eligibility.

66 2/3% of basic monthly earnings not to exceed maximum monthly benefit of $5,000.  Minimum monthly benefit is the greater of $100 or 10% of the monthly benefit before deductions for other income benefits.

Based on gross monthly income.

ACCIDENTAL DEATH & DISMEMBERMENT BENEFIT

Full-time employees:  First of month following eligibility and receipt of enrollment form.

Accidental Death is an amount equal to the in-force life benefit.  Dismemberment benefit according to schedule.   Seat Belt Rider equals $50,000 or the amount of Accidental Death and Dismemberment benefit, whichever is less.

INSURANCE

All Full-time and Regular Part- employees

(20+ Hours Per Week)

First of the month after one calendar month of continuous employment.  Employee and spouse must apply for coverage.

Additional term insurance from a minimum of $10,000 to a maximum of $250,000 in increments of $10,000 for employee and spouse.  During Open Enrollment employee may purchase $100,000 without physical.  Employee may purchase $30,000 coverage for spouse without physical and $10,000 coverage for children.

PENSION

Contributions must be made on the participant's behalf for 60 of the 84 months immediately proceeding retirement.

Retirement benefits under terms of the plan.  5 years vesting.

Approximately 28-30 days per year based on years of service.  Prorated based on hours paid.

28 hours maximum per year. Accrual rate will be 0.01346 per straight time hour worked.
