EMPLOYEES IN AN AUTHORIZED POSITION OF 48 HOURS PER PAY PERIOD OR MORE WILL BE ELIGIBLE FOR THE MEDICAL, DENTAL, VISION, RX, AND HOSPITAL HRA PLANS. 

 

EMPLOYEES WHO WORK 40-47 HOURS PER PAY PERIOD WILL BE NOT BE ELIGIBLE FOR THE MEDICAL, DENTAL, VISION, RX, AND HOSPITAL HRA PLANS, BUT WILL STILL BE ELIGIBLE FOR THE OTHER BENEFIT PLANS, INCLUDING PTO, ESL, SHORT AND LONG-TERM DISABILITY, AND LIFE INSURANCE.

 

BENEFIT: BAH 10/28 MEDICAL PLAN

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management

Full-time and regular part-time employees (48+ hours every two weeks)
When Eligible: First of the month following receipt of enrollment form.
Benefits: Major medical 80%-100%, individual deductible $500, family deductible aggregate $1,500.  Lifetime maximum $1,000,000.

Prescription benefit - Retail $15, $30, or $45 based on formulary; Mail Order - $20 copay
Who Pays: Hospital and employee co-pay.

 

 

BENEFIT:  BAH 10/28 DENTAL PLAN

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management

Full-time and regular part-time employees (48+ hours every two weeks)
When Eligible: First of the month following receipt of enrollment form.
Benefits: $25 deductible per participant.  Annual Benefit $1,000.
Preventative & Restorative:
1st Year - 80% of UCR
2nd Year - 90% of UCR
3rd+ Year - 100% of UCR
Major Services 70% of UCR
Orthodontia benefit at 70% of UCR ($1,500 lifetime).
Who Pays: Hospital and employee co-pay.

 

 

BENEFIT: BAH 10/28 VISION PLAN

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management

Full-time and regular part-time employees (48+ hours every two weeks)
When Eligible: First of the month following receipt of enrollment form.
Benefits: Exam every 24 months ($20 copay)

Lenses & Frames every 24 months:

Single, Bifocal, Trifocal, Lenticular lenses, frames or contract lenses - $150 every 24 months.
Who Pays: Hospital and employee co-pay.

 

BENEFIT: BAH SHORT-TERM DISABILITY

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management
Full-time and regular part-time employees (40+ hours every two weeks)
When Eligible: First of the month after eligibility date.
Benefits: 70% of your weekly earnings to a maximum of $200. Maximum benefit coverage is 25 weeks. Benefits begin on 8th day of illness or injury, or first day of hospitalization
Who Pays: Hospital.

 

BENEFIT: LONG-TERM DISABILITY

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management
Full-time and regular part-time employees (40+ hours every two weeks).
When Eligible: First of the month after eligibility date.
Benefits: 66 2/3% of basic monthly earnings not to exceed maximum monthly benefit of $5,000. Minimum monthly benefit is the greater of $100 or 10% of the monthly benefit before deductions for other income benefits.

BENEFIT: TERM LIFE INSURANCE & ACCIDENTAL DEATH AND DISMEMBERMENT

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management
Full-time and regular part-time employees (40+ hours every two weeks)
When Eligible: Full-time Employees (70-80 hours every two weeks): First of the month following receipt of enrollment form.

Part-time Employees (40-69 hours every two weeks): First of the month after one calendar month of eligibility and receipt of enrollment form.
Benefits: Based on gross monthly income:  $10,000 to $35,000 term life benefit.
Accidental Death is an amount equal to the in-force life benefit; Dismemberment according to schedule.

Who Pays: Hospital.

 

BENEFIT: SUPPLEMENTAL LIFE

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management
Full-time and regular part-time employees (40+ hours every two weeks)

When Eligible: First of the month after one calendar month of continuous employment. Employee and spouse only.
Benefits: Additional term insurance from a minimum of $10,000 to a maximum of $250,000 in increments of $10,000 for employee and spouse.
Who Pays: Employee.

 

BENEFIT: PAID TIME OFF (PTO)

Who Receives: Registered Nurses, Licensed Practical Nurses, non-contract employees and management
Full-time and regular part-time employees (40+ hours every two weeks)

When Eligible: Immediately.
Benefits: Approximately 28 days per year based on years of service. Prorated based on hours paid.
Who Pays: Hospital.

 

BENEFIT: EXTENDED SICK LEAVE (ESL)

Who Receives: Registered Nurses, Licensed Practical Nurses, noncontract employees and management
Full-time and regular part-time employees (40+ hours every two weeks).
When Eligible: Immediately.
Benefits: 28 hours maximum per year. Accrual rate will be 0.01346 per straight time hour worked.
Who Pays: Hospital.

 

BENEFIT: EMPLOYEE ASSISTANCE

Who Receives: All Employees and family members residing in the home.
When Eligible: Immediately.
Benefits: Up to three (3) counseling visits per event.

BENEFIT: UFCW MEDICAL - JOINT LABOR MANAGEMENT RETAIL TRUST

Who Receives: All UFCW Full-time and part-time employees (20+ hours per week)
When Eligible: First day of the fourth month after successfully completing their probationary period (480 hours/90-calendar days), provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of three consecutive calendar months following the probationary period.

Benefits: $150 per person deductible. $450 deductible for family. The plan pays 85% of allowable expenses with PPO Provider; 80% of covered expenses to $5,000, then 100% to maximum with Non-PPO Provider.
Who Pays: Hospital.

 

BENEFIT: UFCW DENTAL
Who Receives: All UFCW Full-time and part-time employees (20+ hours per week).
When Eligible:. First day of the fourth month after successfully completing their probationary period (480 hours/90-calendar days), provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of three consecutive calendar months following the probationary period.  Orthodontia:  The first of the month following nine months of Dental Plan coverage in any 12-month period.
Benefits: Dental:  $1,100 each calendar year.  See Fee Schedule in UFCW Brochure.   Orthodontia: benefits paid at 70% up to a maximum of $800.00 , 5-year maximum.
Who Pays: Hospital.

 

BENEFIT: UFCW VISION (Vision service Plan (VSP)

Who Receives: All UFCW Full-time and Regular Part-time employees (20+ hours per week)
When Eligible: First day of the fourth month after successfully completing their probationary period (480 hours/90-calendar days), provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of three consecutive calendar months following the probationary period. 

Benefits:  Eye Exam: Every 12 months from the last date of service - 100% of VSP Allowable.  $10.00 deductible.

Lenses:  Every 12 months from the last date of service, 100% of VSP Allowable.

Frames:  Every 24 months from the last date of service, 100% of VSP Allowable.

Contacts:  $100 allowance towards cost.
Who Pays: Hospital.

 

BENEFIT: UFCW - DEFINED BENEFIT PENSION

Who Receives: All UFCW Full-time and regular part-time employees (20+ per week).
When Eligible: Contributions must be made on the participant's behalf for 60 of the 84 months immediately proceeding retirement.
Benefits: Retirement benefits under terms of the plan. 5 years vesting.
Who Pays: Hospital.

 

BENEFIT: SHORT-TERM DISABILITY

Who Receives: All UFCW Full-time and regular part-time employees (20+ per week

When Eligible: First day of the fourth month after successfully completing their probationary period (480 hours/90-calendar days), provided that at least 80 hours of employer contributions have been made on the Employee's behalf in each of three consecutive calendar months following the probationary period. 
Benefits: 66 2/3% of your straight-time weekly earnings prior to your disabity.  Maximum benefit is $110.00 a week for first six weeks and maximum of $160 a week for remaining 20 weeks.

BENEFIT: LONG-TERM DISABILITY

Who Receives: All UFCW Full-time and regular part-time employees (20+ per week

When Eligible: First of the month after eligibility date.
Benefits: 66 2/3% of basic monthly earnings not to exceed maximum monthly benefit of $5,000. Minimum monthly benefit is the greater of $100 or 10% of the monthly benefit before deductions for other income benefits.

Benefits: Based on gross monthly income:  $10,000 to $35,000 term life benefit.
